Preparing for your dog's first visit is easy.  Follow the Steps below to complete your registration and help us provide the best possible care.

  .  Complete your client registration:

Your client registration assists in confirming you are in our service area and provides additional information about your pet for review prior to the first visit.  It also provides information about our policies and agreements. Once the registration is received Mamie will reach out to schedule or answer any questions.

..  Ask your veterinarian to Send a Referral:

Mamie believes in a collaborative care model with your veterinarian. A referral allows her to communicate with your veterinarian and coordinate the safest, most effective treatment plan for your dog.


  • If you have been specifically referred to Mamie by your vet or you are just searching for a practitioner because your vet has recommended trying rehabilitation for your dog, you may already have the required documentation. Mamie will help you confirm this during your phone consult.


  • If you have NOT been referred by your veterinarian and are interested in trying rehabilitation, it's important that you first have that conversation with your vet to facilitate the collaboration in your pet's care.
  • We will help facilitate the referral for you.  Just give us a call.
  • You can download a printable referral form to take to your vet to complete and have available at your first visit.
